  • must meet one or more of the following conditions -- need assistance with tasks of daily living, or monitoring of medication, or availability of on site nursing, or must be at risk of being abused or abusing others, and community services are no longer sufficient
How to apply: Call above number for information - home visit may be necessary to complete assessment Placement in long term care homes -- eligible applicants will be placed on waiting list of one to three homes in Ontario chosen by the applicant
