Service description: Community based mental health, rehabilitation and support services, provided in the setting of the individual's choice including home, school or workplace

Rehabilitation Services -- for children and adults with physical, developmental and mental health challenges, through home or community visits
  • includes occupational therapists, physiotherapists, dietitians, social workers and speech language pathologists


Site Support Services -- support to tenants and staff in various housing settings to maintain tenants' health and independence

Case Management Services -- comprehensive intensive case management for individuals with long term mental health problems, including men with histories of homelessness, residents of designated supported housing and adults with acquired brain injuries

Court Support Services -- for individuals with mental health problems in conflict with the law on a minor offence
  • assists individuals suitable for diversion to the mental health system, including links to appropriate community resources and services


Geriatric Mental Health Services -- rehabilitation and case management for individuals 65 years and over with mental health/emotional problems that interfere with independent living, as well as anyone with dementia associated with aging

Network Therapy Services -- for individuals with long term mental health challenges and the people who are part of their social support network

Developmental Services -- for transitional age youth with developmental disabilities

Mental Health and Justice Initiative -- housing and support for persons with serious mental illness, current involvement with the criminal justice system, and homeless or potentially homeless
