- In-home health and social services -- need for professional or support services in the community (home or school) that cannot be met on an outpatient basis * those needing service may include adults and children with any illness, seniors, persons with physical, psychiatric or developmental disabilities
- Placement in long term care facilities -- adults 18 years and over with disabilities, who require placement in long term care facilities * must meet one or more of the following conditions -- need assistance with tasks of daily living, or monitoring of medication, or availability of on site nursing, or must be at risk of being abused or abusing others, and community services no longer sufficient How to apply Call above number for information * home visit may be necessary to complete assessment
- Placement in long term care facilities -- eligible applicants will be placed on waiting list of one to three facilities in Ontario chosen by the applicant * applicants will not be placed without their consent * applicants may select individual facilities, but may not apply to them directly Fees Free with Ontario Health Insurance Physical access Wheelchair accessible building including main entrance and barrier free washrooms Service description Arranges in-home or school-based health and social services, and placement in long term care facilities * service coordination * information and referral for related community services
- In-home services -- nursing care, physiotherapy, occupational therapy, speech-language pathology, social work, nutritional counselling, personal support, homemaking * if eligible for above services, other services may be provided such as drug card, medical equipment or supplies, laboratory services, transportation
- School-based services -- professional services to enable children with disabilities or ongoing diseases to attend school
- Acquired brain injuries -- access to and coordination of services * centralized service for all of City of Toronto provided by Toronto Community Care Access Centre 416-506-9888 Also information and assistance in linking to existing community support services, which may also be contacted directly by consumers * such services include friendly visiting, meals on wheels, volunteer transportation, security checks, that are relatively low in cost and primarily volunteer-based * also help finding other services, such as attendant care for persons with physical disabilities and supportive housing services
- Placement in long term care facilities -- access for admission to all long term care facilities in Toronto, or for transfer to facilities outside Toronto * does not include seniors apartments, retirement homes, or chronic care hospitals * subsidy for ward accommodation may be available -- apply directly to facility after placement
- Short stay (vacation or respite) care -- respite for caregivers * supportive care for those who need to recover strength * includes persons with Alzheimer Disease
- Crisis placements -- when support suddenly breaks down at home, person is placed wherever a bed is available in Toronto, while awaiting placement in the facility of choice
