Alzheimer’s and Dementia Care
Our caregivers are specially trained and certified in dementia care to ensure that we remain on top of the latest advancements, activities, and care plans that can best benefit your loved one and your family. We are also committed to helping educate family members on the best ways to communicate with and care for persons with Alzheimer’s disease. Here to Care for Seniors can offer Alzheimer’s patients and their families the following services; respite care, educational resources, continuity of daily routine, meaningful activities.
We customize our care plans to meet the individual needs of each client according to their discharge plan. Our trained and compassionate caregivers are supervised by registered nurses and provide a safe and peaceful environment in the home that helps facilitate recovery. We offer the following post-hospitalization recovery services: transportation from hospital/to follow up appointments, assistance discharge forms, personal care as well as filling prescriptions. Additional services include, meal preparation, shopping/errands, Post-operative exercise assistance and more.
Our respite care services help you balance your busy life, or find some time to relax and refresh while ensuring your loved one is in professional, experienced hands. Our respite care services include: companionship, transportation, light housekeeping, meal preparation, bathing, and more.

The Anne Johnston Health Station (AJHS) is a not-for-profit community health centre providing a wide range of programs and services that promote the health and well-being of youth, seniors and people with physical disabilities. Seniors Home Health program
The Seniors Home Health Program provides primary health care services to frail and homebound seniors, ages 55 and up, that live in our catchment area. Our interdisciplinary team of providers includes physicians, nurse practitioner, registered nurse, occupational therapist, chiropodist and registered dietitian. The goal of the Seniors Home Health Program is to provide client-centered care that improves seniors’ ability to live independently and stay out of hospital.

Live-In Home Care Program
Live-In Care provides older adults with full-time, around the clock care. Clients and their families can have peace of mind with reliable, live-in caregivers that monitor safety, attend to any immediate or daily needs and provide companionship.
Hourly Home Care Program
By helping with Activities of Daily Living (ADLs), our caregivers enable clients to maintain their normal daily routines. Hourly home care for elderly, gives the client the flexibility to choose the most suitable times for service. Surrounded by their possessions, pets, and familiar environment, our clients receive the support they need and enjoy their regular activities.
Caregivers will assist in all activities of daily living including:
- Laundry and change of bed linens.
- Grocery shopping and errands.
- Companionship and range of motion exercise.
- Bathing, dressing and grooming assistance.
- Transportation to doctor appointments, supermarket, pharmacy.
- Assisting with walking and transfer from bed to wheelchair.
- Status reporting to family.
- Medication reminders.
Cost: Hourly care is billed at one fixed rate at a minimum of four hours, regardless of day of the week or time of day. Our 24/7 care is similarly billed at one fixed rate by the hour, but is usually reserved only for those patients who require close attention at all times. For live-in home care services, there is a daily rate, with no minimum number of days per week to be met.

Loving Support Services goes beyond the expected to get the love and support you deserve in the comfort of your home. We believe in giving quality care and working together to overcome barriers that may hinder you living in your own home. We offer services that aim towards promoting independence and personal growth.
Programs & Services:
Personal Care- Providing assistance to seniors who are unable to properly care for themselves. Personal Care includes assisting with bathing, eating, dressing, toilet, grooming, assist with mobility and moving around the home. It also includes companionship.
Home Help Services
Assisting seniors with household duties, bed making, changing linens, laundry and ironing, taking out garbage, dusting furniture, organize closets, grocery shopping, preparing meals, assist with errands, drop off/ pick-up dry cleaning, pick-up medication, escort to events, plant/pet care and companionship.
Caregivers move in with you to assist you with a variety of services. Keeping seniors active and healthy, conversing in conversation, playing games, watching movies, doing crafts, reading, assist with planning activities or events, providing respite care, appointment reminder or booking appointments, making grocery lists and personal tasks.
Alzheimer & Dementia Care
Helping seniors maintain familiar routines and with being socially engaged with friends and the community. Caregivers assist seniors by following medical instructions and care plans.
Encouraging seniors to keep to their usual life’s schedule by offering transportation services. We’ll come to you and drop and/ or pick you up from your appointments and life’s events.

North York Community House is a dynamic neighbourhood centre offering innovative programs and services to newcomers & residents, helping build strong, healthy communities.
Newcomer Connections for Senior Caregivers
The aim of this project is to identify, engage, and support senior caregivers in northwest Toronto who are at a high risk for social isolation due to language and cultural barriers. We will connect newcomer seniors to peer mentors and youth volunteers to provide information and support in the caregivers’ own languages and provide direct services both in the community and at home.
Friendly Visiting Program
“Mentors” are matched with senior caregivers (“partners”) to provide 1-to-1 support through social interaction, resource-sharing, and community navigation. Based on your schedule, mentors will call or visit once a week for a minimum of 6 months (at least two in-person visits per month).
Yoga classes, English Conversation Circles, computer classes, educational workshops and an 8-week, enjoyable course (Living Life to the Full) to learn strategies to make you feel better!

We began caring for seniors and enabling seniors to care for themselves in 1983. We continue to do so today as an accredited, not-for-profit community support service agency in Toronto by offering a wide range of practical and low-cost services to seniors and their caregivers. Our services help seniors stay safe, connected, and live as independently as possible, as well as prevent premature or inappropriate institutionalization.
Programs & Services
SPRINT Senior Care’s In-Home Services Department provides a range of home care services to promote living at home independently and safely. Their services include homemaking and personal care. Homemaking encompasses light cleaning and grocery shopping, while personal care includes help with bathing, dressing, meal preparation, and medication reminders.
This program provides frail and homebound seniors with physician-led, interdisciplinary care at home. Under the clinical leadership of primary care physician Dr. Mark Nowaczynski, the integrated HouseCalls team includes two additional part-time physicians, a social worker, an occupational therapist, a physiotherapist, and a nurse practitioner, all of whom are supported by a team coordinator. This team provides seniors with comprehensive and ongoing primary care, as well as chronic and acute disease management, medication administration, in-home interdisciplinary assessment, ongoing case management, and system navigation. The HouseCalls team also works actively to facilitate safe discharges and transitions from the hospital, emergency departments and inpatient beds, as well as from transitional and convalescent care.
Meals on Wheels
Meals on Wheels (MOW) program is volunteer-based and delivers tasty, nutritious meals directly to clients’ doors seven days a week, 365 days a year.

MSCG’s mission is to improve the lives and health of seniors, people with disabilities and those suffering from dementia and Alzheimer’s by implementing a variety of programs that improve the health and wellness of seniors. Our objective is to reduce isolation and increase physical and mental activity amongst seniors. Our goals are to alleviate feelings of isolation which are often felt in the socially neglected communities by organizing activities such as congregate dining, friendly visiting, and other social and recreational events.

Home Support – our home support service includes housekeeping, personal care, basic nursing, meal preparations, shopping, banking and escorting to medical appointments to assist seniors and the disabled
Friendly Visiting – To alleviate any feelings of loneliness and isolation, particularly for seniors and the disabled with language barriers, we offer friendly visiting that provides much needed cheerful and meaningful interactions. These interactions help keep the mind active
Transportation – we offer transportation to seniors to pre-arranged medical appointments
