Service description: Choices for Adult Living -- group activities and one-to-one support

• facilitates the successful transition from school to work and community participation
• support with various goals including employment preparation, post-secondary education, leisure activities, volunteer work

Adult Day Support Program -- three days per week

• community based activities
• some one-to-one support available
