Surrey Place Centre provides specialized clinical services that are responsive to individual’s needs and promote health and well-being in the Toronto region.

We help children and adults living with developmental disabilities, autism spectrum disorder and visual impairments reach their full potential. We offer a variety of groups and workshops for clients, families and caregivers, as well as extensive education and consultation services to community agencies.

Adult Program 18+

The Adult Services Program at Surrey Place Centre provides comprehensive clinical services to adult clients over 18 years of age who have developmental disabilities. The Program supports their families, their caregivers and service providers. The age 18 marks the legal age of adulthood and the transition from the Ministry of Children and Youth to the Ministry of Community and Social Services.
The goals of the Program are to help clients to manage the challenges of daily living, promote their independence and enhance the overall quality of their lives.

FJC Refugee Centre
Primary Health Care & Mental Health Clinic

Programs & Services:

FCJ Refugee Centre is providing primary health care clinic, mental health clinic and self-care at their main location to meet the disenfranchised populations from diverse cultural backgrounds unique health needs. The very essence of the program is to focus on the holistic health care which is integrated approach to health that treats individual as “whole”. The holistic model of care does not only provide primary health care but also focuses on, emotional, psychological and spiritual well-being.

FCJ Refugee Centre identifies equity and culturally competent strategies and services as critical to improving and maintaining the health of the vulnerable populations. The clinic focuses on illness prevention and health promotion. This includes, Primary Health Clinic, Mental Health Counselling and Yoga. It is a shift from illness to wellness so that prevention becomes a priority to help individuals to reach their optimum level of wellness and help prevent diseases.


Available for anyone experiencing barriers accessing healthcare.

Women's College Hospital
We advance and advocate for the health of women and improve healthcare options for all by developing, researching, teaching and delivering new treatments and models of integrated care.

Programs & Services:

Cardiology Program
Our Women’s Cardiovascular Health Initiative is a prevention program for women at high risk of heart disease, and a rehab program for women recovering from a heart problem who need to manage this chronic condition.

Dermatology Program
Our dermatology program offers a wide range of services such as Moh’s micrographic surgery for diseases that improve with light and systemic therapies, all types of chronic wound care, as well as seven other speciality clinics including the Melanoma and Pigmented Lesion Clinic, Transplant Clinic, Transitional Clinic, Hair Loss Clinic and The Ethnic Skin and Research Unit.

Diabetes Program
This program offers a multidisciplinary, integrated approach for patients with Type 1 diabetes, emerging adults with Type 1 diabetes (ages 18-25), and complex patients with Type 2 diabetes. The program leads an insulin pump program providing resources and support for patients on insulin pumps.

Gynecology Program
This program offers services to address the following; abnormal uterine bleeding, ovarian cancer, colposcopy, Polycystic Ovarian Syndrome, Turners syndrome etc.

Osteoporosis & Bone Health
We provide cutting-edge treatment and education in new and innovative ways to reduce fracture risk and improve quality of life.

Respirology Program
The Respirology Program is an interprofessional clinical care, research and education program for individuals at risk for or with pulmonary disease. The program involves physicians, respiratory therapists, educators and pulmonary function technologists.

Prenatal & Postpartum
The Prenatal & Postpartum Services aim to provide parents with informed support throughout their pregnancy and postpartum period. These supportive services include Breast Feeding Support, Breast Pump Rental Program, Infant CPR, and Childbirth Education Classes.

Unison Health & Community Services
Working together to deliver accessible and high quality health and community services that are integrated, respond to needs, build on strengths and inspire change.

Programs & Services:

Doctors & Nurses
Nurse Practitioners have additional education and  as well as the above, are able to provide ongoing care for some chronic conditions, order tests, diagnose conditions and order prescriptions for you. The doctors provide ongoing medical care to clients with more complex medical issues.

Counselling & Case Management
Our counsellors offer a full range of therapeutic, clinical and case management services to assist individuals and families who face a variety of life challenges. We work with people experiencing depression or anxiety, marital or family breakdown, post-traumatic stress disorder, and a variety of other complex issues which cause instability and emotional distress.

Diabetes Management:
Individual counselling and group education classes for people with diabetes and pre-diabetes.

Unison’s dietitians counsel clients one-on-one for issues including, but not limited to, heart health, healthy weight, growth and development for babies and children, and dietary management / prevention of diabetes.

Foot Care
The Unison Health and Community Services chiropody program provides treatment, foot care, education and referrals for seniors, people with disabilities, adults and children who have underlying medical disorders, dermatological problems or other foot care needs.

Sherbourne Health Centre
We work closely with our partners and community members. Our primary care and chronic disease management services, health promotion and education, outreach and social supports, and mental health services work together to provide transformative care and support.

Programs & Services:

A Family Health Team is an approach to primary health care that brings together different health care providers to coordinate your care.

Health Bus
The health bus delivers mobile on-the-spot nursing care to homeless and under-housed people in the southeast Toronto neighbourhood, as well as other parts of the city.

Sherbourne offers a wide range of primary healthcare programs and services to lesbian, gay, bisexual, trans, two-spirited, intersex, queer or questioning individuals, such as, preventative health care including routine physicals, vaccinations, screenings etc.

The Sherbourne Infirmary is a short-term health care unit where people of all genders who are homeless or under-housed can stay while recovering from an acute medical condition, illness or injury. The Infirmary is a safe space where clients can rest and recover in a comfortable, supportive environment.

Alzheimer Society Toronto
The mission of the Alzheimer Society is to alleviate the personal and social consequences of Alzheimer’s disease and other dementias and to promote research. The role of the Alzheimer Society of Toronto is to offer support, information and education to people with dementia, their families and their caregivers, to increase public awareness of dementia, to promote research, and to advocate for services that respect the dignity of the individual.

Programs & Services:

Dementia Counselling
Alzheimer Society of Toronto social workers provide free dementia support to anyone touched by dementia in Toronto.

Support Groups
Living with a family member or friend that has been diagnosed with Alzheimer’s disease or other dementia can be a different experience for everyone. Joining a free dementia support group creates an opportunity to learn from your peers and share insights.

Active Living Program
In partnership with Manulife, the Alzheimer Society of Toronto is proud to offer an exciting range of activities to enhance social inclusion, physical well-being and explore the benefits of creative therapies for people with dementia and their care partners.

Music Project
Music can allow a person living with dementia to rediscover pleasure in the world. The Alzheimer Society Music Project was created in response to overwhelming evidence showing the beneficial effects of music and stimulation on people living with dementia. Through the Music Project, personalized playlists are provided to support a person living with dementia and improve their quality of life.

Alpha Health Services
At ALPHA Health Services, we strive to offer the best care, advice, and treatment to enable you to be as physical as you want to be, to get you living an active life, to allow you to reach your peak performance in sports, hobbies or regular life, and to encourage healthy aging. We believe in a holistic approach to rehabilitation and strive to get you back safely and as quickly as possible.

Programs & Services:

Physiotherapy – Physiotherapists are primary health care professionals with a significant role in health promotion and treatment of injury and disease. They combine their in-depth knowledge of the body and how it works with specialized hands-on clinical skills to assess, diagnose and treat symptoms of illness, injury or disability.

Dry Needling – Abnormal tension in muscles causes pain. Dry Needling is a form of acupuncture which focuses on these specific muscles in order to regain normal tension, and thus decrease pain. Dry needling has also proven to be very effective for those suffering from chronic pain.

Personal Training – The goal of personal training is to help you achieve improved performance if you are an athlete, or help you reach your personal fitness goal.

Massage Therapy – At ALPHA Health Services, massage therapy is offered for relaxation, relieving muscle pain, preventing stiffness and aiding in rehabilitation.

Kid E Care
Kid E Care’s goal is to provide exceptional pediatric care with shorter wait times than what patients typically experience at hospital emergency rooms.

Programs & Services:

The Urgent Care Clinic at kid E care can diagnose and treat most childhood injuries and illnesses. Use this clinic for urgent but non life-threatening illnesses or injuries. We see children with cuts, fevers, colds, viruses, minor burns, sprains, strains, simple fractures and other illnesses. Your child will be seen by a paediatrician with experience managing urgent care problems.
