Eligibility
  • Community Support and Research Unit -- clients of the Centre for Addiction and Mental Health
  • Homes for Special Care -- adults over 18 years with a psychiatric diagnosis, who require supervision and ongoing support with daily living activities
  • Employment Support and Development -- persons with history of mental health, addiction or concurrent disorder difficulties

How to apply Professional, hospital, community agency or self referral
  • Community Support and Research Unit -- by referral from a health care or community support professional
  • Ontario Health Insurance Card Kiosk -- three pieces of identification required
